Space Shuttle Columbia Disaster
The Space Hhuttle Columbia crashed over Texas on 1/2/2003 whilst returning to earth, having spent 15 days 22 hours and 16 minutes in space.
During this disaster, 7 crew members perished, amongst them Ilan Ramon, the first Israeli astronaut in space.
Tens of thousands of some of the world's top engineers planned the trip to space, a tremendous amount of calculations. 2 days prior to the incident, they consulted with their meteorologist about the upcoming weather forecast. Not taking in the most important detail, could the event been predicted and consequently prevented?

The claim was that a crack in the wing was caused by damage from a tile.

On the 16th day, the Space Shuttle crashed due to a crack in the wing.

The crew were asked to perform experiments in the field of Physics, Biology and Chemistry.
"Have the gates of death been opened unto thee? or hast thou seen the doors of the shadow of death?
